Welcome to A systems approach to investigating and learning from patient safety incidents 

Course Overview

This CPD course is aimed at those who lead investigations and other learning responses and those in Patient Safety Incident Response Framework (PSIRF) oversight roles. The course meets the minimum training requirement of this specific element of the new PSIRF. This course will close on Friday 24 May 2024. You will only have from the time you enrol to the close date to complete this course.

Learning Objectives

  1. Understand the principles for adopting a systems approach to safety investigation
  2. Recognise the impact of system design on human performance and wellbeing
  3. Be aware of system frameworks and be able to apply the Systems Engineering Initiative for Patient Safety (SEIPS) to an investigation
  4. Understand the need to define the scope of an investigation
  5. Understand the importance of systematically gathering and thematically analysing different forms of evidence
  6. Understand how to engage and involve those affected by patient safety incidents in a meaningful and compassionate way
  7. Understand the importance of collaboratively developing system focused safety recommendations
  8. Understand the importance of writing inclusive investigation reports
